1. Survey of IEP Meeting Facilitation for Parents/Caregivers

This is the first in a series of needs assessments being conducted to coordinate the efforts of special education families and educators. This survey is open to Special Education teachers, ESAs, Para-educators, Principals, Special Education Directors, and families of Special Education students.

We are seeking your input on Individual Education Program (IEP) meetings:

  • When IEP meeting facilitation would be of benefit?
  • How IEP meeting facilitation would best work?

Please take a few moments and complete this short survey. The information you provide will be used by the Special Ed Support Center to ensure that those participating in IEPs have the resources they need.  There are 17 questions in this survey.

2. Survey of School Services

In this survey, we want to know how you feel about your child’s school. Please complete a separate survey for each child with disabilities. Your opinion is important. While the results of the survey will be shared, any information that would identify an individual will be completely confidential. Your responses will help us as we work to:

  • support activities that create productive interactions between school staff and families
  • establish a resource referral system that works for YOU
  • develop a system of training for educators and families to ensure your child's needs are met.
